Horizons Hospice provides in home supportive care from Registered Nurses, Physicians, Home Hospice Aides, Volunteers, Social Work, Spiritual Care and Bereavement to patients and families dealing with a life limiting illness. Horizons understands that each person is unique, knowing that we tailor our care to meet each person’s individual needs.
Hospice covers medication related to the life limiting disease including medication for management of symptoms such as pain, nausea, difficulty sleeping and anxiety. Hospice also provides medical equipment such as oxygen, hospital bed, bed side commode, wheelchair, walker and other supportive medical equipment at no cost to the patient or family. Horizons also provides personal care items for the patient including adult briefs, chux, and wound care.
Horizons Hospice offers a home hospice aide to provide daily assistance with light housekeeping, bathing, feeding, dressing, and other personal care needs. Our nurses make several visits weekly to ensure the best care for your loved one, and are available 24/7 to meet the needs of our patients and families.
