Cornerstone is a locally owned and operated home health and hospice agency. We currently serve Champaign, Clark, and Logan counties, with plans to expand. Our highly trained staff will provide you with exceptional care and compassion in the comfort of your own home. We bring hospital skills such as Skilled Nursing(IV therapy and infusions, wound care, diabetic teaching and observation, post surgery follow-up, chemotherapy infustions, and more), Physical/Occupational Therapy, and/or Speech Therapy right to your door. Our hospice care will provide you and your family with all the support necessary, such as Registered Nurses (available and on call 24 hours a day), medical equipment, prescriptions, and/or bereavement/family support programs. We are Medicaid and Medicare certified, a provider of Medicaid Community Based Waiver Programs, and also accept most insurances.
